As a Hospice Care Transition Nurse (CTN), you will:
- Ensure timely and responsive care by providing daily onsite access to transitional care coordination and ensuring timely assessment of patient and family needs prior to discharge
- Provide information to families and patients to manage care
- Support the hospital staff
- Expedite referrals for any type of post-acute care (home care, hospice, palliative, personal care and support services, skilled nursing facilities&rehab)
- Improve the transition to the home care/hospice teams
- Provide insights on post-acute care trends
To qualify as a Hospice Care Transition Nurse (CTN) with us, you will need:
- Licensure: Current unrestricted license to practice as a Registered Nurse (RN) required
- One (1) year of professional experience practicing as listed above, required
- Reliable transportation to/from care sites and/or work locations
- Previous experience in home care or hospice preferred
- Previous experience in sales/marketing preferred
