Your Purpose in This Role
As a Transitional Care Liaison, you'll be the bridge between Touchstone Communities and healthcare providers, driving sales through value-based selling and building strong relationships. You'll represent our skilled nursing communities and showcase clinical outcomes that matter.
What You'll Do
- Develop and execute a Regional Sales Plan
- Act as a liaison with hospitals, SNFs, and healthcare providers
- Communicate effectively with communities and regional teams
- Manage CRM reporting in Point Click Care
- Ensure compliance with HIPAA, infection control, and company policies
What We're Looking For
- 2+ years of healthcare sales/business development/marketing industry experience
- Proficiency in Microsoft Office; CRM experience is a plus
- Ability to work independently and collaboratively
- Experience marketing Long-Term Care/Post-Acute/Hospital or SNFs in the greater Houston, TX region is required.
