Proactively communicate with patients to reschedule visits and coordinate changes with your supervisor.- **Comprehensive Assessments:** Conduct thorough OASIS and PT assessments of assigned patients, evaluating psychosocial, educational, safety, and risk factors. Ensure all documentation aligns with federal, state, and organizational standards.- **Tailored Care Plans:** Develop individualized plans of care that promote optimal functional independence and safety, with specific goals and logical interventions designed for each patient's journey.- **Skilled Treatment Delivery:** Provide expert physical therapy treatments utilizing a variety of therapeutic techniques. Assess patient status and progress during each visit to ensure high-quality care.- **Collaborative Care Coordination:** Work as a vital team member alongside physicians, nurses, caregivers, and rehab services to deliver seamless patient care. Ensure all communications are documented appropriately.- **Supervision and Guidance:** Lead and supervise the clinical activities of PTAs, PT students, and home care aides, ensuring compliance with care standards.- **Efficient Documentation:** Complete necessary paperwork per regulations and policies to reflect care provided and outcomes achieved. Ensure documentation is compliant and timely.- **Billing Accuracy:** Manage paperwork to support accurate billing processes and address inconsistencies in insurance coverage proactively.- **In-Service Training Participation:** Engage in training programs for various stakeholders including rehabilitation and medical staff, enhancing community knowledge and awareness.- **Continual Learning:** Keep your skills sharp and stay updated on physical therapy standards to offer the best care possible.- **Marketing Engagement:** Participate in promoting Rehab services through specialized services and public health initiatives to expand reach and awareness.- **Comprehensive Health Assessments:** Conduct a variety of assessments including PT/INR and blood pressure to ensure patient health.- **Patient Procedures:** Carry out essential patient-related procedures such as staple/suture removal and wound care with precision.- **Timely Schedule Communication:** Ensure any changes to patient care schedules are communicated promptly to schedulers and supervisors.- **OASIS Compliance:** Fulfill required OASIS time points in line with agency and regulatory guidelines to ensure quality care.- **Quality Improvement Collaboration:** Work together with QI staff to enhance accuracy in clinical notes and documentation.- **Protocol Adherence:** Follow agency protocols, including McKesson processes, to maintain operational efficiency.- **Form Accuracy:** Ensure all necessary agency, state, and federal forms are completed promptly and accurately during patient visits.- **Time-Efficient Visits:** Manage visits effectively to ensure supervision does not overlap with routine appointments.- **HCA Service Coordination:** Assess and coordinate Home Care Aide services as required, providing ongoing supervision as per regulations.- **Safe Discharge Coordination:** Oversee safe patient discharges and ensure all paperwork is processed according to guidelines.- **Incident Reporting:** Timely transmit any patient incidents to the QI Specialist for review and action.- **Peer Review Participation:** Engage in regular peer review audits, providing and accepting constructive feedback professionally.**Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
